2021 Benefits Enrollment Information

November 2 - 16, 2020

The health and well-being of our caregivers is vitally important to us. We couldn’t deliver the exceptional care we’ve become known for without all of you. We want to provide that same level of care to you and your family—and that starts with benefit plans that offer quality care, convenience and cost-effectiveness.

This webpage contains high level benefit information (medical, dental, vision, etc.) that is helpful to review prior to making your 2021 benefit choices. In addition to this webpage, you can reference the personalized newsletter and guide mailed to your home, as well as additional articles, videos, and more on InComm and the HR Portal.

How to Enroll

You must select the benefits you want for 2021 between November 2-16, 2020. If you do not make your elections by the deadline, you will not have any benefits for 2021.

  • To enroll in benefits (medical, dental, vision, etc.), sign into InComm > Community Service Center button > Human Resources tile > mybenefits > Enroll in Benefits
  • To enroll in additional benefits (group accident or critical illness, pet insurance, identity protection, etc.), sign into InComm > Community Service Center button > Human Resources tile > mybenefits > Enroll in Additional Benefits.

2021 Medical Plans

We have three new medical plan choices in 2021. All offer free preventive care with in-network providers, access to high quality medical providers and prescription drug coverage. The difference is in how you pay for healthcare coverage and out-of-pocket expenses for medical services and prescriptions.

Your choices are:

  • EPO (Exclusive Provider Organization) plan
  • HDHP (High Deductible Health Plan) with an HSA (Health Savings Account)
  • PPO (Preferred Provider Organization) plan

How do you know which plan to choose?

The best way to find the plan for you is to compare costs.

1. Look at the numbers

Refer to your open enrollment guide (page 6) or the HR Portal to see how the plans stack up on:

  • Premium: Amount you pay for coverage; deducted from your check each pay period (See your benefits enrollment guide for rates, or log in to the enrollment system starting November 2.)
  • Deductible: Amount you pay out of pocket before the medical plan begins to pay
  • Copays: Set fee for a specific service, like a visit to your primary care physician
  • Coinsurance: When you share the cost of care with the plan; usually done in percentages
  • Out-of-pocket maximum: Most you’ll pay in a calendar year for covered expenses; once you reach this amount, you’re done paying for eligible expenses for the year

2. Ask yourself these questions:

  • Are you meeting your deductible?
  • Do you have any expected healthcare needs in the year ahead that may require more out-of-pocket?
  • Do you prefer a higher premium and a lower deductible, or vice versa?
  • Are there any opportunities to save money?

Prescription Coverage

Prescription coverage is included with each medical plan. Some key things to know:

  • Walgreens Pharmacy and Community Specialty Pharmacy are your preferred in-network pharmacies, where you will always receive the most affordable prescriptions.
  • If you need to use another pharmacy, then the EpiphanyRx* network is your next best option.
  • Choosing generic prescriptions and in-network pharmacies is always the most cost-effective.

Contact EpiphanyRx (844-820-3260) for the list of covered prescriptions, including the Value Based Design medication lists.

*Although CVS is part of the EpiphanyRx network, it is not covered under the Community plan.

Chronic Condition Management

We understand that when you’re dealing with a chronic health issue, it can be challenging and complicated to maintain a healthy lifestyle. We want to help you be in the best position to live well. Our new Chronic Condition Management program (which expands on the existing Bridges to Health programs) will help you and your family deal with select chronic health issues, while also providing discounted medications and services. Our trained staff can also help you navigate your treatment and medication plans if needed. The program specifics are being finalized and more information will be coming soon on the new chronic condition programs.

Disease States

Pulmonary Asthma (moderate or severe), persistent COPD
Cardiovascular Secondary prevention–post heart attack (MI), stroke, stent
Diabetes HgA1C @ 8 or higher
Metabolic Syndrome/Primary Risk Reduction Any combination of hypertension, dyslipidemia, pre-diabetes and/or obesity
Dental Plans

The three plans—Preventive, Intermediate and Premium—are administered by Delta Dental of Indiana.

Some key things to know:

  • All cover in-network preventive care at 100%.
  • You can see any licensed dentist—though you’ll likely have lower out-of-pocket costs when you see a dentist in the Delta Dental network.

Vision Plans

Community offers you a choice of three vision plans: EyeMed, VSP and Davis Vision. They are similar in cost and coverage.

  • All cover a routine eye exam, eyeglasses or contact lenses and frames for each covered dependent.
  • All provide additional discounts on extra pairs of glasses above and beyond the benefit coverage.
  • Plus, you can cover dependent children up to age 26.

Flexible Spending Accounts

Flexible Spending Accounts (FSAs) are a great way to pay for health, dental and vision services and/or dependent care services. They are simple to use, too. Here’s how they work:

  • You contribute pre-tax dollars from your paycheck to your FSA.
  • As you incur eligible expenses, you use your FSA debit card to pay for them.
  • You have the entire year to use your FSA. Any unused FSA dollars are forfeited by March 31, 2022—use it or lose it.

Which healthcare FSA can you have?

  • If you choose the EPO or PPO: Traditional FSA, where you can set aside up to $2,750** for medical, dental and vision expenses
  • If you choose the HDHP: Limited Purpose FSA, where you can set aside up to $2,750** for dental and vision costs only

**2021 contribution limits should be released in late October by the IRS. Any changes to the limits will be shown in the enrollment system.

Dependent Care FSA

You can also choose a Dependent Care FSA, where you can set aside up to $5,000 for eligible dependent care expenses while you and your spouse are actively at work. In addition to traditional daycare for young children, eligible dependent care expenses include latch-key programs and day camps for children under the age of 13.

Life and AD&D Insurance

Community offers basic insurance to provide important financial protection to those who depend on your earnings. This benefit is automatically provided to all benefit-eligible employees. You may purchase additional life insurance for yourself or life insurance for your spouse and any dependents (up to age 26).

Dependent Life

Free coverage at 1X your annual base salary—to all benefit-eligible employees. Accidental death and dismemberment insurance for you and your family. Supplemental life insurance for benefit-eligible employees. Life insurance coverage for a spouse and/or child.
Short-Term and Long-Term Disability

Protecting your salary in the event of disability is very important. That’s why Community offers short-term disability (STD) during the first 13 weeks of disability, after which time long-term disability (LTD) benefits may be available.

Short-Term Disability

  • Provides income replacement if you are unable to work for a short period
  • Benefit-eligible employees may purchase STD
  • Pays up to 70 percent of your base pay*, depending upon your election

Long-Term Disability

  • Provides monthly coverage to protect income for long-term
  • Provided to full-time, benefit-eligible employees at no cost (part-time, benefit-eligible employees may purchase)
  • Provides 50 percent of your base pay automatically for all full-time employees, but you can buy up to 70 percent of base pay.

Additional Benefits

Community gives you the opportunity to select additional benefits. These optional benefits are designed to help you further protect your family and loved ones.

  • Group Critical Illness Insurance (Aflac): Lump sum benefit to help cover your daily living expenses or pay for your out-of-pocket medical costs
  • Group Hospital Indemnity Insurance (Aflac): Cash benefits paid directly to you to help pay for hospital bill, day-to-day expenses, etc.
  • Group Accident Insurance (Aflac): Cash benefits to help cover your out-of-pocket expenses associated with an accident or injury
  • Legal (Hyatt Legal Plans): Basic legal assistance, such as family/ personal law, financial, home and real estate civil lawsuits, future/estate planning, identity management, etc.
  • Pet Discount Plans (Pet Benefit Solutions): 25% savings on all in-house medical services, plus wholesale pricing on brand-name prescriptions and preventives
  • Pet Insurance (Nationwide®): Cash back for vet bills, including accidents, illnesses, hereditary conditions and more
  • Whole Life Insurance (Transamerica): Financial support for families after the death of a loved one
  • Auto and Home/Renters Insurance (Multiple Providers): Savings on auto, home and renters insurance, plus coverage discounts for boats, mobile homes, renters, utility trailers, motorcycles, etc.
  • Identity Protection (Allstate): Insurance policy that includes monitoring of your online activity (from financial transactions to social media) to detect and prevent against fraud
